國立中山大學(以下簡稱本校)為校務行政所需蒐集、處理及利用您的個人資料,依據個人資料保護法規定,告知下列事項,請詳閱:
一、蒐集之目的:瞭解畢業生在職場的發展及畢業後就業狀況,作AACSB評鑑及後續課程規劃之用。
二、蒐集之個人資料類別:識別類(例如:姓名、國民身分證統一編號、電話號碼、行動電話、通訊及戶籍地址、電子郵遞地址、單位、職稱與姓名)、特徵類(例如:出生年月日、國籍)、受僱情形、財務及符合蒐集目的之各項個人資料類別等。
三、個人資料利用之期間、地區、對象及方式:
(一)期間:個人資料蒐集之特定目的存續期間、依相關法令規定、契約約定或本校因執行業務所必須之保存年限。
(二)地區:本國。
(三)對象:本校生與本校畢業生。
四、您可依個人資料保護法,就您的個人資料權利:查詢或請求閱覽、複本、補充、更正或請求停止蒐集並刪除。
五、個人資料之提供:您若拒絕提供個人資料,本校將無法提供相關服務,亦可能無法維護您的權益。若您提供錯誤、過時、不完整或具誤導性的資料,而損及您的相關權益,本校將不負相關賠償責任。

National Sun Yat-sen University Personal Data Collection Statement and Consent Form
 
National Sun Yat-sen University (hereinafter referred to as "the University") collects, processes, and uses your personal data for the requirements of university administration. In accordance with the Personal Data Protection Act, the following information is provided for your review:
1. Purpose of Collection: To understand the career development and employment status of graduates for the purpose of AACSB accreditation and subsequent curriculum planning.
2. Categories of Personal Data Collected: Identification (e.g., name, ID number, phone number, mobile number, correspondence and permanent address, email address, unit, job title), Characteristics (e.g., date of birth, nationality), Employment status, Financial status, and other categories of personal data consistent with the purpose of collection.
3. Period, Region, Target, and Method of Personal Data Use:
(1) Period: The duration of the specific purpose for which the personal data is collected, as stipulated by relevant laws and regulations, contractual agreements, or the retention period necessary for the University's operations.
(2) Region: Domestic (Taiwan).
(3) Target: The University and its students and graduates.
4. Your Rights: In accordance with the Personal Data Protection Act, you may exercise the following rights regarding your personal data: inquiry or request for review, request for copies, supplement or correction, or request to cease collection, processing, use, or deletion.
5. Provision of Personal Data: If you refuse to provide personal data, the University will be unable to provide relevant services and may not be able to maintain your rights and interests. If you provide incorrect, outdated, incomplete, or misleading data that results in damage to your rights, the University shall not be held liable for compensation.

(A)管理學院畢業生離校問卷填寫登入

(1)管院應屆畢業生或延畢生皆需要填寫才可完成離校手續,博士生不須填寫此問卷。
(2)問卷只能填寫一次,送出後如果出現該代碼無效或該代碼已經使用過表示已經填過,不可再登入作答。
(3)底下請輸入你的身份證字號(英文不分大小寫),系統會自動判斷你的身份及系別然後開始填寫問卷。
(4)填寫完成後,底下有填答結果列印請將結果列印出來後送系辦核章確認離校程序。
(5)如你是提前畢業的畢業生或其他該填而未填或無法填,請聯絡 AACSB 辦公室做例外處理。
